Qualifying Conditions for Survivor's Benefit

SECURING YOUR FUTURE
Statement
  • After the death of a member, FNPF is guided by his/her last valid nomination for the distribution of his/her benefit
  • Payment is made as follows :
    • To person/s nominated in a valid nomination filed with FNPF.
    • Where a person nominated, other than the widow, is under the age of 18 years at the time of payment after death, his portion of the amount payable shall be paid to the Public Trustee of Fiji for the benefit of such nominated person.
    • Where there is no nomination filed, payment is transferred to the High Court of Fiji for payment in accordance with the law for the time being in force.
    • Where a nomination is invalid, payment is transferred to the High Court of Fiji for payment in accordance with the law for the time being in force.
A nomination becomes invalid :
  1. By the death of the nominee or where there is more than one nominee, by the death of the nominees in the lifetime of the member.
  2. By marriage after the date of the nomination.
  3. By a subsequent nomination lodged by the member.
Advice of Death
An advice of death may be filed by the deceased’s next of kin, preferably the spouse, or one of the children of the deceased, or parent in case the member is unmarried by completing the prescribed form FNPF 9B2 and submitting certified copies of the following documents pertaining to the deceased member. Before you fill this form read the explanatory notes at the reverse and answer all the questions and submit all the information requested.
  • FNPF membership card ( if available).
  • Death certificate issued by the Registrar General’s office.
  • Birth certificate.
  • Marriage certificate OR If never married, a statutory declaration confirming the deceased’s marital status OR
  • If divorced, a statutory declaration confirming the deceased’s status after divorce with a certified copy of the decree absolute for that divorce. Name & address of the deceased’s last employer and the date last employed. The informant will be advised within 10 working days if claim forms have been sent to nominees or funds are to be paid into the High Court of Fiji
Nominees claim forms
Where the member has filed a valid nomination, the claim form/s (FNPF 9B) is/are sent to each nominee under separate mail through the informant of advice of death. Claim forms are sent through the informant unless the nominee/s have separately advised FNPF of the death and FNPF have their addresses for correspondence from that advice of death.

Claim form/s will be sent out to nominee/s within 10 working days of receipt of the advice of death.

The nominee/s must return their claim form/s duly completed together with a certified copy of their birth certificate. Payment will not be made until all nominees claim forms are returned.

Method of Payment
Nominees who are eligible for a pension will be sent a Pension Option Form (9OP), detailing the available payment options. Options are as follows:
  • Full lump sum
  • Life pension
  • Part lump sum and part life pension
Nominees who are not eligible for pension will be paid a lump sum payment.
Timeframe for processing payment of an application

Pensionable cases
  • Payment will be processed on receipt of the pension option form duly completed.
  • Payment will be made within 3 days of receipt of the 9OP where the member opts for full lump sum payment.
  • Payment will be made within 10 working days if a member opts for any type of pension.
  • Where a pension option form is not returned within 2 months of the date sent, it is deemed that the member has opted for a lump sum payment.
  • All registered and approved applications must be processed within the timeframes in accordance with this policy.

Supporting document(s) to confirm the relationship of informant to the deceased, deceased members birth certificate, marriage certificate or statutory declaration to state that he/she was never legally married
Phase 2 – Beneficiary Process
Completed Claim – 9B Form
Birth and/or Marriage Certificate of Beneficiary
Supporting document(s) to confirm the relationship of beneficiary to the deceased
Valid Bank Statement of nominee
Important Notes
  • Sole nominee if he/she is the widow/er of the deceased can opt for pension
  • Loss of pension benefit if the dependant passes on
  • Processing claims is done only when all claim forms are received
  • Where no claim is made within 12 months of being so paid to High Court, the Court shall repay the money to the Board and the Board shall credit it to the general reserves of the Fund
