$2000 Death Assistance through Special Death Benefit
 
Guidelines
1.     The next of kin of the deceased member can apply for partial payment of SDB payout if deceased member is eligible for SDB. Full SDB premium should have been deducted from member’s account for the year.
2.     Payment shall be made in accordance with Memorandum of Administration filed by the member.
3.     Memorandum of Administration form shall be valid at time of death.
4.     In the absence of a valid Memorandum of Administration payment shall be made according to the full withdrawal policy where only one of the following:
MEMBER’S SPOUSE if the member is legally married or remarried and spouse is still alive;
MEMBER’S PARENTS if the member was single or single parent with minor children or married but  
 separated or divorced;
MEMBER’S ADULT CHILDREN if the member’s spouse and parents are deceased or if the member
 was divorced and separated from his/her living spouse;
MEMBER’S ADULT SIBLINGS if the member was single or single parent with minor children and
 parents are deceased or if the member is married with minors or no children and spouse and parents
 are deceased, or if the member was divorced with minor or no children and parents are deceased.
5.     Maximum of $2,000 will be given under this scheme.
6.     Assistance is for funeral expenses of the deceased member. This assistance cannot be accessed for  any other expenses related to the member.
 
Documents to be Submitted with the Application
 
1.     The member must apply using application Form 9B2 & 9B4.
2.     Certified copies of the Notice to the undertaker or Medical certificate of cause of death, stamped and signed by the hospital medical superintendent/SDMO prior to burial/cremation.
3.     Police Order for Burial only when assistance is sought before date of burial.
4.     Death certificate issued by Registrar of Births and Deaths if burial has taken place as in the case of outer islands.
5.     Evidence of relationship of deceased to the applicant.
6.     Applicant’s original birth certificate.
7.     Deceased member’s original birth certificate.
8.     Completed ‘Additional Information’ Form.
9.     Evidence from the nominated person’s banker confirming the member’s open bank account number for payment deposits. Evidence should not be more than a month old.
 
Period of Entitlement
The next of kin is to apply for this assistance within 20 working days from time of death.
 
Method of Payment
Payment is made direct to the applicant’s bank account.
