SECURING YOUR FUTURE
LOCAL AND OVERSEAS MEDICAL ASSISTANCE

Where a member or his/her spouse, parents and children require medical treatment locally or overseas due to an urgent medical condition, and has produced satisfactory evidence of the same, the Fund may permit the member to withdraw part of the funds in his/her account to assist with the related expenses subject to medical partial eligibility.
 
Guidelines 
  1. The assistance is for the member, spouse, parents, children and brothers and sisters.
  2. The amount applied for must be within the member’s medical partial eligibility.
  3. Where the member’s eligibility is less than 50% of the cost of the full treatment, the Fund may request evidence of other funds available to meet the shortfall before releasing any FNPF funds.
  4. Where the treatment is not available at government hospitals, a specialist is to provide a referral letter for admittance to private hospitals.
  5. Medical assistance will be provided to those conditions that cannot be treated locally, subject to the submission of the necessary reports from the insurance company or specialist report.
  6.  Assistance may also be extended for incidental expenses for overseas medical treatment to a maximum of $5,000 per patient per calendar year.
 Documents to be submitted with Application (Local Treatment)

  1. The member must apply on the prescribed application Form (W30/3).
  2. Evidence of relationship of the patient to the member.
  3. Recent detailed medical report from a registered local doctor.
  4. Breakdown of cost of treatment from medical institution where the patient is undergoing treatment
  5. Evidence from the member’s banker confirming the member’s open bank account number for payment deposits. Evidence should not be more than a month old.
Documents to be Submitted with Application (Overseas Treatment)
  1. Evidence of relationship of the patient to the member.
  2. Referral from patient’s local doctor for overseas treatment.
  3. Confirmation letter from the overseas hospital on patient’s treatment, date of admission, length of stay and breakdown of cost of treatment.
  4. A letter confirming and detailing medical coverage from the insurance company where the patient has a medical insurance cover.
  5. Certified copies of the patient’s passport pages showing details of passport holder and the validity of the passport.
  6. Letter from employer confirming leave granted if patient is currently employed.
  7. Visa to cover period of medical treatment is to be produced before payment is released. (Approval will be granted, but payment processing will be subject to a copy of the visa submitted to the Fund).
  8. Evidence from the member’s Bank confirming the member’s bank account number for payment deposits.
  9. Evidence from the Medical Institutes confirming the Institutes Telegraphic details OR postal address for Bank draft
     

Method of Payment 

  1. Payment will be made to the member and or medical institute / insurance companies depending on the urgency of funds to be paid before the treatment and arrangements between the member and the institute.
  2. Payment overseas will be made by bank draft or telegraphic transfer and the related bank fees will be paid for by the member from the funds withdrawn.
