What are the information and supporting documents required to register as a FNPF member?

Completed FNPF 3 form with information and documents below:

Personal details as stated in member's Identification Card/passport;
Left thumb impression;
Employer's number;
Signature of member;
To be witnessed by the employer/employer's representative/authorised FNPF officer only;
Member's address; and
Photocopy of member's Identification Card/Passport.
